The case history of a 47-year-old woman is described who presented with a rapidly progressive destructivearthritis of both femoral heads, similar to that seen with avascular necrosis and who was discovered in addition to have familial hypercholesterolaemia associated with tendon xanthomata. Details of the family are presented and the relevant literature on the association between hypercholesterolaemia and bone and joint lesions is reviewed. As far as the authors are aware, no other case of this type has been previously reported.
