EXTENDED-HEEL SHOES

DAVID P. EVANS

Department of Rheumatology, University Hospital of Wales Heath Park, Cardiff CF4 4XW

Postural backache is extremely common especially in young women and produces a nagging, low-grade ache, often persistent for many years. It is associated with weak abdominal muscles causing forward pelvic tilt which in turn leads to increased lumbar lordosis. Symptoms can be helped by isometric abdominal exercises.

An extended-heel shoe has been invented which enables patients to exercise their abdominal and quadriceps femoris muscles isometrically. In addition it permits relaxation of back extensor muscles intermittently during prolonged standing. A pilot study has shown significant reduction in the depth of lumbar lordosis during the wearing of this shoe, compared to a control shoe, during six weeks of treatment. All cases, although of a selected type, benefited subjectively. The possible mechanisms of action are discussed.
